Transformation Model of Care

Our Residential Program

The principles that guide our model of care are centered around principles found in the Holy Bible. The driving force is a Christ-like love that is informed by complex trauma. Our model of care allows us to consider any adult biological female for admission into our program that has a history of being forced, deceived, manipulated or threatened into performing a commercial sex act. This includes those needing emergent, supportive or restorative care.

We realize that there are many therapeutic approaches that have been successful and the type of programming that works for one survivor may not work for another. We have integrated our knowledge about relational trauma and sexual trauma into our policies, procedures and practices. We train and equip our staff and volunteers in trauma informed care so as not to re-traumatize the survivors in our program. We attempt to provide a sense of safety for every resident, understanding that this is a very individualized and subjective need. We strive to build and maintain trust with a consistent program. 

 

We encourage peer support and empowerment through celebration of resiliency, strength building and offering choice. We incorporate individualized programming within the context of Biblical trauma-informed relationships. We assist our residents by aiding in emotional regulation, encouraging new patterns of thinking and giving them new coping skills to handle the long-lasting effects of trauma.

Emergent (approx 1-7 days)

In this phase of care, we seek to defuse the immediate threat to your life.

  • We understand that entering our home, that includes community living, can be difficult.

  • We will assess your need for emergency medical care and/or medical detox.

  • We will explain the layout of the home, safety measures that are in place, and our commitment to your confidentiality.

  • We will address life threatening issues.

  • We will offer spiritual support, understanding that you may have been previously hurt in this area.

  • You will be given the opportunity to decompress.

  • You will not be required to engage in structured programming.

    *The services provided will be shelter, clothing, food and emergency medical care as well as coordination with law enforcement if needed.

Stage 1

Safety (1-3 months)

In this phase of care, we will seek to establish safety understanding that it is a key component to healing.

  • We will attempt to help you manage harmful thought patterns and behaviors.

  • We will NOT dive into trauma work during this stage.

  • You will be provided an individualized plan of care.

  • You will begin learning new healthy coping skills.

  • You will receive substance use recovery support.

  • You will experience horse assisted therapy.

  • You will begin our collegiate style education program, Transform U.

  • You will begin exploring options for the next phase of care.

*The services provided will be a continuation of basic needs, with the addition of dental and vision assessments, legal support, spiritual and relational support and assessment for stabilization services.

Stage 2

Stabilization (3-6 months)

In this phase of care, you will begin to rebuild your life through case management support.

  • We will pay attention to your strengths so that we can develop a “strength-based” strategy for your continued growth as your collaborative partner.

  • You will begin a faith-based trauma curriculum with frequent opportunities to practice healthy coping skills.

  • You will be educated about the effects of abuse and trauma.

  • You will begin the process of academic assessment and evaluation so that we can help you meet your education goals.

  • You will begin to dream again.

 

*The services provided will be a continuation of basic needs, legal support, spiritual and relational support with the addition of exploring long-term restorative care services.

Stage 3

Restoration (6 months - 2 years)

In this phase of care, the focus will be encouraging independence by allowing opportunities for community involvement, education advancement, and job readiness.

  • We believe that maturity, not length of time in programming, should guarantee responsibilities and privileges.

  • We will assist you in moving from a survivor mindset to a thriver mindset.

  • You will be assisted on your academic journey.

  • You will navigate career choices and prepare to meet your goals.

  • You will learn how to make healthy social connections outside of the program.

  • You will have access to financial integrity and money management courses to help ensure economic stability.

 

*The services provided will be a continuation of basic needs, legal support, spiritual and relational support with the addition of education and vocation support.

Stage 4

Graduate Resident/Intern

This is the independent phase of care and emphasis will be on personal agency and accountability.

  • This will only be available to those who have graduated from the Transformation Garden or the Transformation Home.

  • This will be a time of transitional living within the facility.

  • You will have to meet requirements to be considered.

  • You must have minimal case management needs.

  • We will provide accountability through supervision of schedule, finances, church attendance and random drug screens.
     

*The purpose of this stage of care is to promote independence while still providing community and accountability.
